How to get a medical marijuana card in Ohio.

Free one-on-one help from a Bloom Cannabis patient rep.

With an Ohio medical card, you can shop from 18, skip the 10% adult-use excise tax and buy as a registered patient. The state charges nothing, the physician visit can happen from home, and a Bloom rep will walk you through every step for free — usually starting the same day.

About a minute. We never ask about your health.

What you get

What an Ohio medical card gets you.

You don't need a card to shop at 21. But with one, three things change at the register — starting with your first patient visit.

Age to shop

18 and up

A medical card is the only way to buy at an Ohio dispensary before you turn 21.

Tax at the register

No 10% excise

Patients pay sales tax only — 6.5% to 8% at Bloom, instead of 16.5% to 18% on adult-use.

How you shop

Check in as a registered patient and shop the patient menu and pricing at any Bloom in Ohio.

What it takes

Less hassle than you'd think.

Most of the work happens on a video call and a state website. If anything stalls, we help — free.

$0 to the state

Ohio dropped its registration fee in 2024. Your only cost is the physician visit, typically $50 to $100.

Done from home

Certifying physicians can see you by telehealth, and the state registry is fully online. No state office visit.

No application to file

If you qualify, the physician submits your registration. You confirm your details and activate it.

Free help if you stall

A Bloom patient rep walks you through any step by phone or email. No health questions, nothing to buy.

Card or no card

What waiting costs you.

Without a card, every adult-use purchase carries Ohio's 10% excise tax. Here's what that adds up to over a year, based on what you spend before tax.

Spend $50 a month

$60 a year

in adult-use excise tax you'd stop paying as a patient.

Spend $100 a month

$120 a year

in excise tax — more than the typical physician visit.

Spend $200 a month

$240 a year

in excise tax, every year you shop without a card.

Turn it around: with the physician visit at $50 to $100 a year, a card pays for itself on tax alone once you spend about $500 to $1,000 a year — roughly $42 to $84 a month. And the day your registration is active, your next purchase at Bloom skips the excise tax.

  • If you're 18 to 20, waiting means not buying at an Ohio dispensary at all until you turn 21.
  • If your card lapses, you're back to adult-use pricing and the excise tax — and under 21, back to not buying. Read how Ohio medical card renewal works.

If you only shop now and then, the saving may not cover the visit. Tell a rep what you usually spend and they'll tell you honestly whether it's worth it. For the full breakdown, see what you'll pay at each Bloom.

Where are you now

If any of these fit, fill in the form so your rep knows where to start. Not sure? Fill it in anyway — they'll figure it out with you.

“I'm ready — where do I start?”

How to find a certifying physician in Ohio, and what to have ready for the visit.

“I'm stuck on the state registry.”

You have the recommendation, but the account won't activate or the email never came.

“Mine's expiring or already lapsed.”

What renewal needs, and how to get back on the registry before you lose patient pricing.

How it works

Four steps, and where people get stuck.

None of this happens at a dispensary. A certified physician recommends you, and the state issues the card. What we do is keep you moving when a step stalls, so you get to patient pricing sooner.

1

Find a certifying physician

Only physicians with a certificate to recommend from the State Medical Board of Ohio can recommend you. Many offer telehealth visits. Snag: not knowing which providers are legitimate, or what records to bring.

Telehealth or in person
2

Get the recommendation

If the physician confirms a qualifying condition, they submit your registration to the state for you. You don't apply yourself. Snag: waiting on a state application you were never meant to file.

Your physician files it
3

Activate your registry account

The state emails a link to confirm your details and activate your registration. There's no fee. Snag: the email lands in spam, or your ID details don't match the record.

Download your card

Your card is digital. Bring it with your valid government-issued photo ID to any Bloom Cannabis store in Ohio and check in as a patient — your first patient purchase skips the excise tax. Snag: arriving with an ID that doesn't match the registry.

Do you qualify

Ohio's 26 qualifying conditions.

This is Ohio's list, not ours. Bloom Cannabis doesn't decide who qualifies and can't tell you whether you do — only a certifying physician can make that call. Check the list before you pay for an appointment.

Before you ask

Ohio medical card questions.

What does Bloom Cannabis get out of helping me?

We hope you'll shop with us afterwards. That's it. We don't certify patients, we take no commission from any physician or certification service, and there's nothing to buy. If a card doesn't make financial sense for you, a rep will say so.

Will you ask about my medical condition?

No — not on the form and not on the phone. We're a dispensary, not a medical provider. Whether you qualify is between you and a certifying physician, and we have no reason to keep a record of your health information.

Do I need a medical card to shop at an Ohio dispensary?

No. Anyone 21 or older with a valid government-issued photo ID can buy adult-use products at any Bloom Cannabis store in Ohio. A card is only required if you're 18 to 20, or if you want to shop as a registered patient — patient menu and pricing, and no 10% adult-use excise tax.

How much does an Ohio medical card cost?

The state charges nothing to register. The physician visit is the real cost, and it repeats every year when you renew. It typically costs $50 to $100, but prices vary by provider, so compare a few before you book.

How much can a medical card save me?

Patients skip Ohio's 10% adult-use excise tax, so you keep about $10 of every $100 you spend before tax. At Bloom's Ohio stores, patients pay 6.5% to 8% in sales tax instead of 16.5% to 18% in total. A card covers a $50 to $100 physician visit once you spend roughly $500 to $1,000 a year. If you spend less than that, the tax saving alone may not cover it.

Can I do it all online?

Yes. Ohio law lets certifying physicians recommend patients through a telehealth visit, and the state registry is entirely online. You never need to visit a state office.

How long does it take to get approved?

The physician submits your registration after the visit, and the state then emails you a link to activate it. Once your account is active, you can download your card and shop. If the state flags something to fix, you have 90 days to correct it before the registration goes stale.

Can I get a card if I'm under 21?

Yes. Ohio residents 18 and older can register as patients, and a card is the only way to buy at an Ohio dispensary before 21. Patients under 18 can register with a parent or legal guardian as their caregiver, but they can't buy cannabis themselves — their caregiver does.

What if my card already expired?

You lose patient status at the register — and if you're under 21, the ability to buy at all — until you renew. Renewal needs a follow-up visit with a certifying physician. See how Ohio medical card renewal works, or ask a rep to walk you through it.

Does Ohio accept out-of-state medical cards?

No. Ohio has no reciprocity, and its patient registry is for Ohio residents. Visitors 21 and older can buy adult-use cannabis with a valid government-issued photo ID, and it has to stay in Ohio.
